Lag when picking up first item of game
I've noticed this the past few times I've played Doomsday 2.0.1 (not sure for earlier versions), but when I start a new game, there is a brief lag when picking up the first item of the game (doesn't matter what the item is).
If you hit F7 to end the game and then start a new game, it doesn't happen. If you quit the game (going back to the Doomsday home page) and start a new game, this lag happens.
Most of the time it isn't a big deal, but it can throw you off if you start off a level surrounded by enemies and need to make a quick move, like picking up the Shotgun in E4M1 of Ultimate Doom.
My PC specs are Windows 10 pro, an i7 3770, NVIDIA 660 Ti, and 16 GB of RAM, if that matters.

It is definitely on my list of things to improve once I get to reworking the internals of the resource subsystem.
