How can I set up an Icarus -server?

The shell only asks for the game wad -folder.
How can I tell the server to start the DoomII addon "icarus" and make that as a server?

    You can specify add-on files to load from Advanced Settings ⇒ Options. The wiki contains articles on using the Shell and a reference guide for the available Server command-line arguments, should you need any more info.

    You'll want something like: 
    -file "c:\path\to\my\doomwads\icarus.wad"
